For this team Discussion, you will have the opportunity to review the work of others and provide them with peer-reviewed feedback, as well as consider how their continuum of care might be improved. Further, you will be able to compare and contrast the other countries identified by your team members, outside of the United States, that are recognized for organizing care around medical conditions.
In this Discussion, you will continue to analyze your continuum of care scenario using the value-based care model, the IOM Aims, and a country comparison. You will be placed into a team and share your suggestions for improvements. Then, you will provide peer-review feedback on your colleagues’ suggestions, and they will do the same for you. This feedback will be incorporated into Week 6 Assignment, which you should continue to update through Week 6.
In Weeks 4–6, your team (“Team A”) will be composed of people who were assigned to the same health issue, although it is likely that you each will have chosen different diagnoses. In Week 6, you will incorporate the peer feedback into an Improvement Plan Assignment. In Weeks 7–9, you will be placed into a different team (“Team B”), which will be based on similar geographic regions. Similar to Week 6, you will again incorporate peer feedback into an Improvement Plan Assignment in Week 9.

To prepare for this Discussion:
- This week, your Instructor will assign you to work with a team (Team A) that has been assigned to the same health issue. You will continue to collaborate with this Team A through Week 6.
- Communicate and plan with your team to ensure that each member receives peer-review feedback from at least two colleagues. You can create a separate Discussion thread to discuss, and it is recommended to designate which members will provide feedback to whom. Additionally, consider varying the individuals who provide feedback in Week 5 and Week 6 so that each member can receive more diverse perspectives in the feedback they receive.
- Locate your Community Profile and Continuum of Care document that you developed in Week 3 and return to your continuum of care scenario. Research how your community and specific diagnosis relate the following:
- The value-based care element of: Organize care around medical conditions.
- The IOM Aim of: Be patient centered.
- Additionally, research and identify a country outside of the United States that has a healthcare delivery model that closely aligns with the value-based care element of “organize care around medical conditions.”

Post your evaluation of a continuum of care scenario and country comparison based on your Week 3 Community Profile and Continuum of Care scenario, to address the following:
- In one sentence, briefly identify the city/state in which your health community is located, the specific diagnosis for your identified health issue, and the country you used as a comparison.
- Using at least one scholarly source, propose at least one improvement that could be made based on the following element of the value-based care model:
- Organize care around medical conditions
- Using at least one scholarly source, propose at least one improvement that could be made based on the following IOM Aim:
- Be patient-centered
- Using at least one scholarly source, compare your community’s model of healthcare delivery to the delivery model of the country that closely aligns with the value-based care element of “organize care around medical conditions.” What lessons—positive or negative—could your community learn from the other country?
Note: Be sure that your proposals and comparisons are supported with evidence outside of the textbook, which could include the other Learning Resources from this week or other scholarly sources.
Read the postings from each member of your team, even if you are not intending to provide peer-review feedback to each member. Consider how your colleagues’ postings relate to the information presented in the Learning Resources and to your own posting.
Refer to the Week 4 Discussion Rubric for specific grading elements and criteria. Your Instructor will use this grading rubric to assess your work.
